1. Download or purchase a custom background 
2. Login to your Twitter account
3. Click Settings then go to the Design tab
4. Click Change Background Image at bottom

5. Then click the Choose File button
6. Choose the file you downloaded on step 1
7. Click Save...Done!

Many ways are actually illegal on Twitter and can get you banned. The good honest way is to manage the hell out of your followers. 
I know a couple small businesses and a writer that have tried different techniques and marketing to get a mass of followers. They agreed TWEETBIG to be most effective. Supposedly, this management system is the rage with top bloggers, but my business buddies liked it as well. I guess it helped them get a nice volume of word of mouth sales.
